Rosie O’Donnell explains why she finally went public about secret facelift

Rosie O’Donnell has opened up on why she decided to go public with her recent face lift.

Last month, the comedian shared the news that she’d undergone the procedure in an unusually frank Substack post.

In that post, O’Donnell, 64, said she once felt ‘strongly’ against facelifts, viewing them as a ‘betrayal’ of feminism and of getting older.

But her perspective shifted after she lost 50 pounds, which she said changed what she was seeing when she looked at her face.

“It wasn’t wrinkles – it was gravity,” O’Donnell penned. “I’d look in the mirror and think – this isn’t aging, this is melting with intention.”

O’Donnell explained that she had the surgery in January, choosing a doctor who had previously treated friends of hers.

She also acknowledged feeling conflicted about the decision, writing about the discomfort she felt in sharing it publicly.

She went on to write: “I don’t own anyone any explanation but its feels familiar; the sense of deceit I’m struggling with.”

With that in mind, she said she decided to be upfront because she ‘never liked secrets’ and wanted to address it head-on.

Since then, the Now and Then star has offered more context on what motivated her to speak out about it.

“Authenticity is the goal in these days and times,” she told E! News at the 2026 Tony Awards. “And I think all that matters is truth and love, and so I wanted to be truthful and say all the complicated emotions I had about it.”

She also described how her daughter initially responded when O’Donnell raised the idea of surgery.

“My little autistic 13-year-old said to me, ‘If you do it, I would not be able to respect you ever again,'” O’Donnell said. “And that put me off it for a few months, but then when I went and did it in January, I came home 10 days later, and they never noticed.”

O’Donnell’s comments arrive at a time when more public figures are choosing to be open about cosmetic procedures rather than dodging the topic.

One moment that helped fuel the wider conversation came when Kylie Jenner replied to a TikTok last year, sharing specifics about her breast augmentation after a creator asked for details.

The TikToker said she was thinking about a similar procedure and wanted to know exactly what Jenner had done.

The mom-of-two replied to the video: “445 cc, moderate profile, half under muscle!!!!! Silicone!!! Garth Fisher!!! Hope this helps lol.”

Not long after, Olympian Simone Biles also shared that she’d undergone three separate cosmetic procedures, including a lower blepharoplasty.
