A Tesla driver died on an Arizona freeway after the electric vehicle halted in live traffic while its automated driving system was active, newly released federal filings show.

The 2020 Tesla Model 3 was traveling eastbound on Loop 202 in Mesa around 3 a.m. On October 31 when it came to a complete stop in the middle of the highway. A Ford F-350 pickup truck slammed into the rear of the motionless sedan, and the Tesla driver was pronounced dead at the scene, Arizona’s Family (KPHO/KTVK) and 12News had reported at the time.

Initial local coverage of the crash made no mention of automated driving technology. However, documents that Tesla later submitted to the National Highway Traffic Safety Administration confirmed a darker explanation. According to the crash report, an automated driving system — either the company’s standard Autopilot or its premium “Full Self-Driving” (FSD) software — was “verified engaged” during the collision. The vehicle’s pre-crash movement was listed as “stopped – 0 mph.”

The Arizona fatality was not an isolated incident. Electrek reported that just weeks earlier, another Model 3 running on Tesla’s driver-assist software had stopped in the middle of a Florida highway before a fatal rear-end collision killed that driver as well.

Tesla possesses the critical data that could explain why the Arizona vehicle halted, including video footage, telematics, and the event-data recorder. Yet the company redacted three key fields in its NHTSA filing: the crash narrative, the software version, and whether the vehicle was within its approved operating area. Tesla designated this information “confidential business information,” a classification the company applies to 99.9% of its crash reports.

The deadly halt fits a documented pattern of malfunction that has shadowed Tesla’s automated systems for years. The company has long struggled with a “phantom braking” glitch in which vehicles suddenly slam on the brakes for obstacles that do not exist. The NHTSA opened a preliminary investigation into the phenomenon on February 16, 2022 following a surge in consumer complaints about rapid deceleration at highway speeds while using Autopilot or adaptive cruise control.

That four-year federal probe concluded in July 2026. The agency noted a sharp drop-off in incident reports and found no underlying mechanical brake defect or unreasonable collision risk.

The Arizona crash and its Florida predecessor raise fresh questions about whether the NHTSA’s closed investigation adequately addressed the dangers of sudden stops in live highway traffic. With Tesla withholding the technical specifics of both incidents under claims of business confidentiality, outside investigators have limited ability to assess whether the same software flaw caused both fatalities.
