Taylor Swift debuted her star-laden “Patient Zero” music video at the 2026 MTV Video Music Awards.
The 36-year-old pop star premiered the short during Sunday’s ceremony after teasing it earlier in the week on “CBS Mornings,” where a 13-second clip revealed that Dakota Johnson, 36, and Colin Farrell, 48, lead the cast. Swift directed the project herself and enlisted Oscar-winning cinematographer Emmanuel Lubezki to shoot it.
Johnson had already raved about the experience at the “Verity” red carpet premiere in London last Thursday. “That was just the best,” she gushed. “Working with your friends is like the most fun thing on the planet.”
“We’ve known each other for so long, we respect each other’s creative minds, and to be able to work alongside her and support and assist and be a person whom she tells what to do is wonderful,” Johnson added.
The video casts Swift as the ghost of Farrell’s murdered former wife, invisible to the living as she tries to warn his new bride, played by Johnson. The two women first appear side by side at a graveyard in matching gray outfits. Swift shadows Johnson through her daily life, singing warnings about a toxic ex, and even comforts her as she vomits in a bathroom — sickened, the viewer learns, by her husband’s poison.
The narrative intensifies when Farrell’s character flirts with a woman played by Cara Delevingne, followed by a violent argument between husband and wife while Swift’s ghost looks on. The three converge at a gala where Johnson’s character finally sees Swift and collapses. Forced to leave, Farrell berates his wife for causing a scene during the car ride home.
“I thought you were different,” he snaps, leaving her bewildered. “Different from who?”
The answer arrives in a flashback: Farrell’s character had poisoned and killed Swift’s character, and the grave Johnson visited at the video’s opening belongs to her predecessor. The new marriage mirrors the old one down to its deadly details.
Swift chose the VMAs stage for this reveal strategically. The ceremony also honored her with the inaugural MTV VMA Artist Director Honors, recognizing “groundbreaking music artists whose work behind the camera has expanded music videos and advanced visual storytelling.” The award made her the most-awarded solo artist in VMAs history. Johnson presented it to her, and the two watched the “Patient Zero” premiere together from the crowd.
The single belongs to “The Life of a Showgirl: The Encore,” a four-song expanded edition of her 2025 chart-topping album that she announced Wednesday and released Friday. The additional tracks are “Cleveland!,” “Pink Clouding” and “Babylon.”
Swift had seeded hints about the project for months, including in her “Opalite” music video and across her social media platforms. She also tucked Easter eggs into a surprise pre-recorded Emmys 2026 cameo with the cast of “Law & Order: Special Victims Unit,” and confirmed the single’s release Tuesday with a social media announcement.
Johnson’s participation arrives amid continued tabloid interest in her personal life. The outlet recently reported that the actress had been using Swift’s New York City apartment for meetings with Machine Gun Kelly, her latest rumored romantic interest.
Swift’s next scheduled public appearance is her ongoing “The Life of a Showgirl” world tour, with the next stop set for Toronto.
