NJ and NY Rank Among the Most Expensive States to Give Birth as Costs Keep Rising

New mothers in New York and New Jersey face some of the steepest childbirth bills in the nation, with median in-network costs running thousands of dollars above the national average, according to fresh data from the nonprofit FAIR Health.

The median price tag for a vaginal delivery with in-network providers hit $23,000 in both states, while a C-section climbed to $27,000, the organization found. Those figures place the Mid-Atlantic neighbors among the priciest places in America to welcome a baby, trailing only Alaska’s stratospheric rates.

Alaska topped the cost chart at $31,000 for vaginal birth and $42,000 for C-section, FAIR Health reported. California hospitals billed mothers’ insurance plans a median $21,000 for vaginal delivery and $25,000 for cesarean births.

The national picture looks somewhat less dire, though still substantial. The median in-network cost across all states stood at $15,700 for vaginal delivery and roughly $20,000 for C-section, according to the nonprofit’s analysis.

Those nationwide averages mask sharp increases in recent years. Costs jumped about 3.6% from last year alone, but the longer trend is more dramatic: vaginal birth prices have surged 21% since 2022, while C-section costs have leaped 28% over the same period, FAIR found.

The billed amounts cover a bundle of services including consultations, anesthesia, labs, ultrasounds, and other facility and professional fees. Crucially, these represent sticker prices that providers send to insurers, not what insured families typically shell out themselves.

Going out-of-network sends costs spiraling far higher. The national median for an out-of-network C-section reached $43,000, with Nevada posting an eye-watering $79,000 median for the procedure.

For women with employer-sponsored insurance, the actual out-of-pocket hit averages a more manageable but still significant $2,700 for pregnancy, birth, and postpartum care combined, according to health research organization KFF.

Geography shapes the financial burden dramatically. The South and Midwest offer the most affordable options for budget-conscious families. The Dakotas recorded the lowest out-of-network costs nationally at just under $21,000 each for vaginal births. Alabama and Mississippi claimed the cheapest in-network rates at just over $10,000.

The pricing data lands against a troubling demographic backdrop. American birth rates are declining and fertility rates have already slipped below replacement levels, meaning the population would shrink without immigration.

FAIR Health, an independent nonprofit that manages a massive database of health insurance claims, regularly publishes cost analyses drawn from billions of billed medical procedures. The organization’s figures are widely cited in policy debates over healthcare affordability.

The 2022 baseline for FAIR Health’s comparison predates the most intense period of post-pandemic inflation, suggesting the cost escalations may reflect broader economic pressures hitting hospital operations, staffing, and supply chains.

Health economists have long noted the opacity of American maternity pricing, where identical services can vary by tens of thousands of dollars depending on hospital, insurer negotiation leverage, and state regulatory environments. The new data underscores how that variability falls hardest on coastal and rural states with limited provider competition.
